Create a form by adding a One-Line Text Box

Your text box that collects the email address should be named email. Double-click on the text box to bring up the Text Box Properties dialog. Change the name to email.

While you cursor is located anywhere within your form open up the Form Properties Dialog.

Erase the entry in the File name field. Enter your email address in the E-mail address field. Then click on the Options... button.

For E-mail format select the format for your email. You may want to test the various options and select the one you like best. In the Subject line field enter the subject line you would like to have on the emails when you receive them.
Now here's the most important part. Enter the name of your email text box in the Reply-to line field. In this example it is email. If you don't do this you can't automatically reply to emails sent to you from your web site.
